循環器・腎・代謝疾患


循環器・腎・代謝疾患治療のパイオニアとして

循環器・腎・代謝疾患は、世界中で多くの人々の健康や命に大きな影響を与えています。しかしながら、これらの疾患に対する診断や治療は依然として十分ではなく、疾患の相互関係もあまり知られていません1-7

アストラゼネカは、1964年に前身の英国ICIが世界初のβ遮断薬を開発する等、循環器・腎・代謝疾患領域におけるパイオニアとして、世界をリードしてきました。卓越した研究開発⼒とパイオニアとしてのDNAを原動⼒とし、診断・治療の面でいまだ多くの課題が残る循環器・腎・代謝疾患領域において、サイエンスの限界を超える挑戦を続けています。

特に日本の新たな国民病と呼ばれることもある「慢性腎臓病(以下、CKD)」8においては、「CKDの克服」というミッションを掲げ、早期診断・早期治療介入の普及を通じて、この疾患に新たなパラダイムシフトを起こすことを目指しています。



循環器・腎・代謝疾患が世界にもたらす影響と有病率

    は、2040年までに循環器・腎・代謝疾患になると予測されています9

    の循環器・腎・代謝疾患患者さんが、そのうち2つ以上の疾患を患っています10

    の2型糖尿病患者さんが少なくとも1つの他の循環器・腎・代謝疾患を患っています11

  • 40~50%

    の心不全患者さんがCKDを患っています12

日本では上位10死因のうち2位が心疾患、4位が脳血管疾患、8位が腎不全と報告されており13、循環器・腎・代謝疾患患者さんの多くが治療を必要としています。

アストラゼネカの目標は、これらの複雑に絡み合う循環器・腎・代謝疾患の治療をさらに進歩させ、治癒につなげていくことです 。

慢性腎臓病


慢性腎臓病(CKD)について

CKDは、様々な原疾患や進行段階を含む多くの慢性的な腎疾患の総称です15。CKDは、私たちが直面する最大の健康課題の1つであり、世界中で10人に1人が罹患しています16。日本においても、成人の5人に1人、約2,000万人のCKD患者さんがいると言われています17

CKD患者さんは徐々に腎機能が低下し、最終的には腎不全へ至る可能性があります18。CKDの一般的な原因は、糖尿病、高血圧、糸球体腎炎です18。CKD患者さんのほとんど(約90%)は、自分がこの病気であることに気づいていません19, 20。また、腎機能が重度に低下している人(CKDステージ3~5)の約半数は、症状が現れているにもかかわらず診断されていません21

腎臓は、毒素や老廃物を取り除き、血圧をコントロールし、体液、塩分、ミネラルのバランスを調整し、体内の多くの臓器を最適な状態に保つという重要な役割を担っています22, 23。そのため、腎機能の低下はこれらのバランスの崩れにつながります24, 25


高カリウム血症について

高カリウム血症は、血液中のカリウムイオン(K+)濃度が正常よりも高い状態となる病態で、治療が必要となる場合があります。また慢性の高カリウム血症においては、その治療が長期にわたることもあります26, 27。腎機能が低下するとカリウムを排泄できず、体内のカリウム濃度が増加する場合があります24, 25。CKDと心不全を合併する患者さんの最大48%で、高カリウム血症が認められたという報告もあります28

高カリウム血症の多くは無症状ですが、無治療のまま放置すると、不整脈や心停止等の心血管系合併症を引き起こすことがあります27


心不全


心不全について

心不全とは、心臓が悪いために、息切れやむくみがおこり、だんだん悪くなり、命にかかわる病気です29。世界中で6,400万人以上が心不全に罹患しており、65歳以上の主な入院原因となっています30, 31。多くの場合、関連する複数の疾患を合併していることから、個々の心臓の潜在的な疾患要因を深く理解する必要があります。

日本の心不全患者さんは約120万人と推計されています32。また、心不全の罹患率は高齢者ほど高くなることが知られており、高齢化が進み続ける日本においては、2030年には130万人に達すると推計され「心不全パンデミック」と呼ばれることもあります32

現在、心不全に対しては「画一的な」治療アプローチが取られていますが、心不全の発症メカニズムは多岐にわたるため、この画一的な治療はすべての患者にとっての最適な治療とは言えないことがあり、患者さんの最大50%は診断から5年以内に重篤な経過をたどるとする報告もあります33


心血管疾患


心血管疾患について

心血管疾患は、世界で約5億2,300万人が罹患しており、多くの場合、心血管系の健康状態が低下しています34。心血管疾患は世界的に死因の第1位であるだけでなく35、他の心血管に関する疾患とも密接に関連しています36

日本では、300万人を超える心血管疾患の患者さんがいると推計されており37、日本人の死因2位となっています13

心血管疾患のうち代表的なものに冠動脈疾患(狭心症・心筋梗塞等)があり、これらは心臓に血液を供給する冠動脈が狭くなる慢性的かつ持続的な疾患です38, 39。これらの疾患では脂肪物質、コレステロール、細胞老廃物等の蓄積物であるアテローム性動脈硬化プラークが、冠動脈に蓄積して心筋への血流を妨げます39。このプラークが破れた場合、心筋への血液供給が部分的あるいは完全に遮断され、心臓発作を引き起こす可能性があります38, 39。また、約3人に1人の慢性冠動脈疾患患者さんが2型糖尿病を併存しており40、冠動脈疾患と2型糖尿病を併存している患者さんでは、2型糖尿病単独の患者さんと比べて重大な心血管疾患のリスクが2倍になります41

心筋梗塞を起こしたことのある人々では、その後、心血管の健康状態が悪化する場合が少なくありません。心臓発作を起こした人の30%以上が、1年以内に再発作、脳卒中等により重篤な経過となるリスクがあることから、早期診断や、冠動脈疾患の進行を軽減または阻止するための治療法の開発、制御できていない危険因子に対する治療アプローチが重要であると強く言われています42。なお、CKD等の危険因子も、心臓発作等の心血管疾患を引き起こす可能性を高めます43


抗凝固作用の中和


心血管疾患患者さんの緊急時のケア

日本含め、世界中で多くの人々が抗凝固薬、特に直接作用型経口抗凝固薬(以下、DOAC)を服用しています44。DOACは、不整脈(特に心房細動)による脳梗塞予防や、血栓塞栓症の治療・再発予防等を目的として、多くの患者さんに使用されています44-47。心血管疾患は依然として世界中で死因の上位を占めており、DOACの使用は今後もさらに増加し​​続けると予想されています47

DOACは血液が固まる働きを抑え、心臓等、体内の様々な臓器に影響を及ぼす可能性がある血栓の形成を防ぐことを目的として使用されます。一方で、服用中は通常より出血が起こりやすい状態となるため、事故や転倒等のきっかけで、頭蓋内・消化管等での大出血につながるリスクが高くなります45, 46, 48

毎年、DOACを服用する人の2~4%が大出血を発症するという報告があり49、その原因は、自然発生的、外傷、侵襲的処置による合併症等、様々です50。抗凝固薬服用中の患者さんにおいて大出血が発現した際には、出血の増大を抑えることが重要です50。大出血の処置には、迅速かつ効率的な対応、そして診療ガイドラインに基づいた明確なプロトコルが必要です51


代謝


糖尿病について

糖尿病患者さんの数は、2045年までに世界で7億8,300万人に増加すると予想されており、日本においては予備群も含め約2,000万人の糖尿病患者さんがいるといわれています52-56

肥満とインスリン抵抗性、インスリン分泌不全は2型糖尿病発症の主な要因であり、2型糖尿病は、心臓、循環器、腎臓などの疾患の発症リスクを高めます57。また、2型糖尿病の発症はさらなる生活の質の低下、医療費の増大にもつながります52-55
