AstraZeneca PLC(本社:英国ケンブリッジ、最高経営責任者(CEO):パスカル・ソリオ[Pascal Soriot]、以下、アストラゼネカ)は、直接作用型経口第Xa因子阻害剤(アピキサバンおよびリバーロキサバンなど)服用中に頭蓋内出血を起こした患者さんを対象として、オンデキサ(一般名:アンデキサネット アルファ、以下:オンデキサ)の有効性と安全性を評価する製造販売後第IV相試験(製造販売後臨床試験)であるANNEXA-I試験を早期に終了することをお知らせします1。この早期終了の決定は、オンデキサが通常の治療と比較して優れた止血効果、すなわち生命を脅かす可能性のある脳内出血の拡大を阻止する効果において、事前規定された試験の終了基準を達成したことに基づいています1,2。
本試験は、450例の患者さんを無作為化し、1カ月間の追跡調査を実施した後に予定されていた有効性の中間評価の結果を受けて、独立したデータおよび安全性モニタリング委員会(DSMB)が試験終了を勧告しました。この中間評価の結果では、当初想定されていたよりも試験登録の早期に、抗凝固作用に対する中和というオンデキサによるベネフィットが示されました。
カナダ王立内科医協会特別会員(FRCPC)であり、公衆衛生研究所(Population Health Research Institute)の上席研究員兼カナダのオンタリオ州に位置するマックマスター大学の名誉教授であるStuart J. Connolly医師は次のように述べています。「標的とする抗凝固作用の中和により、通常の治療と比較して出血コントロールの改善を示し、この試験が予定されていた中間解析の有効性評価項目を達成したことを喜ばしく思っています。さらに詳細な解析により、有効性および安全性の最終的な結果を共有できることを楽しみにしているとともに、得られたデータが生命を脅かす可能性のある出血の治療に関する新たなガイダンスへの道を開いてくれると期待しています」。
アストラゼネカのバイオファーマ研究開発部門担当エグゼクティブバイスプレジデントであるMene Pangalosは、次のように述べています。「世界中で何百万人もの人が有害な血栓の発生を予防するために第Xa因子阻害剤を使用していますが、これらの薬剤には頻度は低いながらも急性大出血の可能性を高めるという重大なリスクも存在します。私たちは、速やかな治療が必要とされる場合において、第Xa因子阻害剤の活性を特異的に中和し止血に役立つ、効果的かつ信頼性の高い治療として唯一承認されている本剤を提供できることを誇りに思っています」。
オンデキサは、生命を脅かす出血または止血困難な出血の発現時に、直接作用型経口第Xa因子阻害剤の抗凝固作用を速やかに中和するよう特異的にデザインされています3。本剤は、第Xa因子阻害剤であるアピキサバンまたはリバーロキサバン投与中の成人患者さんの治療薬として、米国においてはAndexxaとして迅速承認制度による承認を受け、欧州連合(EU)、スイスおよび英国においては、オンデキサの販売名で条件付き承認を受けています。また、日本でも、直接作用型第Xa因子阻害剤(アピキサバン、リバーロキサバンまたはエドキサバントシル酸塩水和物)の抗凝固作用に対する中和剤として、オンデキサの販売名で承認されています。オンデキサの使用は、複数の領域において、15を超える国内および国際ガイドラインで支持されています4-19。
アストラゼネカは今後、ANNEXA-I試験の終了手続きを開始し、米国およびEUにおける完全承認への切り替えを目指して規制当局への申請手続きを進めてまいります。有効性および安全性の詳細な結果については、今後の医学学会および論文にて発表する予定です。

生命を脅かす出血について
世界中で何百万人もの人々が、血栓形成のリスクを管理するために第Xa因子阻害剤を使用しています20。これらの薬剤は人々の健康とウェルビーイングを維持するために重要ですが、頻度は低いながらも急性大出血という重大なリスクも存在します21-26。止血困難な出血はさまざまな形で発現する可能性があります。放置しておくと生命を脅かすリスクが高いものに頭蓋内出血(ICH)が挙げられます27-29。第Xa因子阻害剤を服用中の患者さんにおいて、大出血は生命を脅かす可能性があり、体内で出血が起こり目に見えないこともあるため、特異的中和剤へのアクセスには差し迫ったニーズが存在します。第Xa因子阻害剤の処方が増加するにつれて、止血困難な出血に対する有効かつ信頼性の高い治療法の必要性も高まっています30,31。
ANNEXA-I試験について
ANNEXA-I試験は、アピキサバンおよびリバーロキサバンなどの直接作用型経口第Xa因子阻害剤の投与を受けていた頭蓋内出血の成人患者さん(18歳以上)を対象として、アンデキサネット アルファの有効性および安全性を通常の治療と比較検討するためにデザインされた、無作為化、多施設共同臨床試験です。また、本試験は米国およびEUにおける完全承認に必要な製造販売後臨床試験の一環として実施されています1。ANNEXA-I試験には、直接作用型経口抗凝固薬(DOAC)の一種である第Xa因子阻害剤(アピキサバンおよびリバーロキサバン)、いわゆる抗凝固薬の投与を受けていた、頭蓋内出血の成人患者さん450例以上が登録されました。本試験の主要評価項目は、オンデキサを投与した場合を4因子含有プロトロンビン複合体濃縮製剤などによる通常の治療と比較した、効果的な止血率です1,2。 ANNEXA-I試験は、通常、血腫の出血規模と部位によって評価される急性頭蓋内出血(ICH)患者さんを対象として実施されました1。血腫のサイズと拡大の測定方法は確立されており、止血効果を結論付ける評価が可能です32,33。
オンデキサについて
オンデキサ(一般名:アンデキサネット アルファ)は遺伝子組換えタンパク質であり、 第Xa因子阻害剤に結合し、その抗凝固作用を速やかに中和するようデザインされています34。オンデキサは、血液凝固に関与する酵素であるヒト第Xa因子の改変タンパク質です。 オンデキサは、第Xa因子を標的として結合し抗凝固作用を示す経口および注射の第Xa因子阻害剤にデコイ(おとり)として作用することで効果を発揮します。オンデキサは、第Xa因子阻害剤関連の出血を起こした患者さんに静脈内投与されると、第Xa因子阻害剤に高い親和性で結合し、第Xa因子活性の阻害を妨げることで、第Xa因子阻害剤による抗凝固作用を中和します。
